Licensed Vocational Nurse (LVN) – Home Health (Office/Field Hybrid)

Job Summary

We are seeking a detail-oriented and compassionate Licensed Vocational Nurse (LVN) to support our Home Health team in an office/field hybrid role. This position focuses on clinical coordination, documentation, communication with patients and providers, and ensuring high-quality care delivery in compliance with state and federal regulations.

Responsibilities

Coordinate and support home health services under the supervision of an RN or Clinical Supervisor

Review, process, and maintain accurate clinical documentation (OASIS, care plans, orders, visit notes, etc.)

Communicate with physicians, therapists, caregivers, patients, and family members regarding care plans and orders

Perform patient follow-ups via phone or telehealth as needed

Assist with admissions, recertifications, discharges, and care coordination

Ensure compliance with Medicare, Medicaid, and agency policies and procedures

Monitor patient progress and report changes in condition to the supervising clinician

Requirements

Current and active LVN license in good standing (state-specific)

Minimum of 1 year of nursing experience; home health experience preferred

Detail-oriented with strong critical thinking skills

Preferred Qualifications

Previous office-based or field home health experience

Bilingual (a plus, but not required)
