We are currently seeking a Community Health Worker (CHW) to join our growing team. This role is critical in supporting Medi-Cal members through outreach, care navigation, and connection to community resources. The ideal candidate has strong community ties, lived experience, and a passion for health equity.

What You’ll Do:
Engage Medi-Cal members through outreach and relationship-building
Assist with care coordination, referrals, and appointment follow-up
Connect members to community resources addressing social needs
Provide culturally responsive health education and support
Document services accurately and timely per Medi-Cal requirements
Collaborate with care managers, social workers, and clinical supervisors

What We’re Looking For:
High school diploma or GED
Community Health Worker (CHW) certificate from a recognized training program (DHCS-aligned preferred)
Experience working with underserved or Medi-Cal populations
Strong communication and documentation skills
Ability to work in the community and under clinical supervision
