RN – In-Home ECM Health Needs Assessment (Orange County)

We are a community-based nonprofit organization serving CalOptima members in Orange County through the Enhanced Care Management (ECM) program. We are seeking an experienced Registered Nurse to complete comprehensive, in-home ECM Health Needs Assessments and Care Plans.

This role is not task nursing. It requires strong clinical judgment, clear problem identification, and audit-ready documentation.

🩺 Scope of Work
• Conduct a comprehensive in-home ECM Health Needs Assessment
• Identify and clearly articulate medical, functional, and psychosocial problems
• Develop SMART, patient-centered goals
• Create a complete care plan with specific, defensible interventions
• Ensure documentation supports clinical rationale and compliance
• Obtain member agreement/consent
• Submit a final RN-signed, defensible care plan

This assessment is detailed and time-intensive. Quality and accuracy matter more than speed.

💰 Compensation
• $200 per completed assessment
• Rate includes documentation and travel time
• Paid only for complete, submission-ready work

⏱ Schedule
• Flexible scheduling
• Visits arranged based on your availability
• Local Orange County visits only
• Ideal for experienced ECM RNs seeking per-diem or supplemental work

✅ Required Qualifications
• Active California RN license
• Direct experience completing ECM Health Needs Assessments and Care Plans
• Strong narrative documentation skills
• Ability to create clear, defensible problem statements, goals, and interventions
• Comfortable charting independently using a laptop/tablet
• Excellent bedside manner and patient-centered communication
• Detail-oriented and compliance-minded
