Adult Day Health Care of Mad River is currently hiring for a Full Time RN. As a member of a multidisciplinary team, you will play a crucial role in providing quality healthcare services to a wide variety of patients. Responsible for administering patient care, collaborating with healthcare professionals and ensuring the well being of individuals under your care. Responsibilities include the management of a small case load of patients for Enhanced Care Management which may include home visits to assess needs and provide caregiver training, and working to connect patients to needed resources. Mon - Fri work week, 8:30 am - 4:00 pm, no weekends, no major holidays. Vacation, sick time and retirement benefits available.
