Job Description
Overview
ADHC center is a Community based Adult Services (CBAS) licensed program that provides services to eligible older adults and/or adults with disabilities to restore or maintain their optimal capacity for self-care and delay or prevent inappropriate or personally undesirable institutionalization.
We have several are hiring RD: Pomona, El Monte & Orange county (coming soon).
Minimum 20-26 hours per month on each location.
RESPONSIBILITIES AND DUTIES INCLUDE BUT ARE NOT LIMITED TO THE FOLLOWING:
1) The dietitian will be included as part of the Multidisciplinary Team when the physician has indicated a special diet for the participant. An individualized treatment plan will be developed and modified as needed based on change of status.
2) Record and sign treatments in the records of those participants who are receiving dietitian services on the day of service.
3) Submit a monthly report of activity and progress notes for those participants receiving dietitian services. The dietitian will also provide status documentation in the quarterly reports for those participants being followed by the dietitian.
4) Review and initial all evaluations and the discharge summary for those participants receiving dietitian services.
5) Interface with other team members, the participant's personal physician, referral agencies, and significant others when appropriate.
6) Carry out appropriate assessments, reassessments and treatment interventions within scope of practice.
7) Ensure that meals and between meal nourishments meet the regulatory requirements in quantity and frequency.
8)Meet with the meal service provider to ensure that all regulations are met.
9)Provide in-service education to staff related to dietary and nutritional needs of the participants. Participate with Nursing Services in educating staff in personal hygiene and infection control practices related to food preparation and services.
10) Provide dietary and nutritional education and counseling to participants as indicated in individualized plan of care.
11) Assist in menu planning for meals and nourishments and ensure therapeutic diets are available as ordered.
12) Comply with all health and safety standards including licensing, fire and disaster preparedness regulations and agency policies, and monitor staff compliance.
13) Conform to all applicable policies of the Center and attend the orientation program offered by the Center.
REQUIREMENTS:
1) Current registration.
2) Liability Insurance.
3) Verification of current certification in first aid and cardiopulmonary resuscitation.
4) Health record from my physician completed within the past 12 months stating that I have no condition that would prohibit my fulfilling the responsibilities and duties of this contract.
5) Evidence of negative screening for tuberculosis within the past 12 months.
