One-on-one nature of home health.
Provide skilled nursing care, including wound care (acute, chronic), trach care, and catheter care.
Administer medications and monitor for side effects.
Perform patient assessments, check vital signs, and monitor health status.
Implement the plan of care as initiated by the Registered Nurse (RN).
Educate patients and family members on disease management and safety.
Document care accurately using Electronic Medical Records (EMR).
Communicate changes in condition to the Clinical Team Manager or physician.
