Licensed Vocational Nurse (LVN) – Adult Residential Facility (ARF)

Position Summary:
The LVN provides nursing care to up to four adult residents in an Adult Residential Facility. This role is not in a Skilled Nursing Facility or Assisted Living. The LVN works with caregivers to support resident health, administer medications and treatments, monitor conditions, and communicate with the care team and physicians.

Job Responsibilities:
• Perform nursing assessments and monitor resident health
• Follow physician orders and report changes in condition
• Administer medications and treatments per policy and orders
• Monitor for health changes; report to supervisor, provider, or call 911/ER if needed
• Document care and maintain electronic health records, including eMAR
• Reconcile medications and coordinate refills
• Communicate with doctors, specialists, and care team about resident needs
• Assist with scheduling and attending appointments
• Transport residents for appointments, outings, and activities as assigned
• Support caregivers with Activities of Daily Living (ADLs)
• Follow infection control, safety, and emergency procedures

Minimum Qualifications:
• Active California LVN License
• Current First Aid and CPR Certification
• Clear DOJ/FBI background check
• Good physical health, TB clearance, and physical exam
• Ability to lift, push, and pull 25+ lbs
• Valid California Driver’s License and reliable transportation
• LVN experience preferred; new graduates welcome
