Job Description

Role of the DSP

The primary role of the DSP is to support individuals with intellectual and/or developmental disabilities to grow and maintain the individual’s desired quality of life. The DSP provides support with daily living and social skills, behavioral support, and community integration according to the individual(s) person-centered plan of support.

Duties and Responsibilities:
• Supports in the implementation of individual program plan for supports and behavior support activities and desired outcomes.
• Provide direct support and skill building supports to individuals within their home and community.
• Performs personal care tasks, to include assistance with basic personal hygiene and grooming, toileting, feeding, mobility, and administering of prescribed medications, etc., as needed.
• Foster positive and collaborative relationship between DSP and individual(s) served.
• Provide encouragement and motivation to individuals
• Ensures client safety and helps to maintain a safe environment.
• Reports observation and concerns of changes in client’s health or physical condition to supervisors.
• Complete daily progress notes and other documents as required.
• Performs needed home management functions such as light housekeeping, laundry, bed making, preparing meals, and cleaning.
• Accompany individuals to scheduled appointments as needed.
• Provide transportation & accompany individuals with running errands, community integration, etc.
• Support individuals with problem solving, decision making and acquiring general information.
• Comply with DBHDS, HCBS, and Agency requirements and regulations
• Other duties as assigned.




Education/Minimum Requirements:
Minimum high school diploma or GED.

Desired qualities of a Staff Member Ideal candidates will possess the following qualities:
• Good oral and written communication skills
• Knowledge of person-centered planning and documentation
• Flexibility, organization, patience, compassion

Experience Preferred:
At least 6 months related experience with individuals with ID/DD and/or mental health diagnosis (i.e. home care, group home, direct care, etc.) preferred.

Licensure:
● Complete a criminal background check/ CPS Central Registry check
● Tuberculosis screening
● Valid state driver’s license and current auto insurance
● CPR/First Aid Training Certification and TOVA training (within 15 days of employment)
● Complete DSP orientation training/testing and Home and Community Based Services (HCBS) training/testing (within 15 days of employment) and other required trainings
● Complete DSP competencies as required and HCBS trainings at least annually

Hours: Hours as specified by Program Manager/Director.
Compensation Package: The DSPs salary is to be negotiated with Program Manager/Director at Commonwealth Center for Family Services, Inc.
